Temporary Coaching Card
Any coach is eligible to apply for a Temporary USA Hockey Coaching Education Card through the District Coach-in-Chief. This may be accomplished by the following procedures:

  1. The coach or coaches must apply for the Temporary Coaching Card through the District Coach-in-Chief. The coach or coaches must use the Temporary Card Request Form, and there must be one form filled out per coach.
  2. Forms are available on our Form page at www.mohockeyyd.org/PDF/Temporary_Card_Request_09.pdf   
  3. Each completed Temporary Card Request Form must be accompanied by a $50.00 check made payable to USA Hockey.
  4. Forms should be mailed to:

  5. A coach can only request a Temporary Card once during their USA Hockey coaching career.
  6. Temporary Cards will not be granted after December 31st of the current coaching season.
  7. The Temporary Card is only good through May 30th of the current hockey season.
  8. All coaches who request a Temporary Coaching Card are still responsible to fulfill the coaching education requirements for the following season. Failure to complete the necessary requirements the following season results in the forfeiture of all coaching credentials.
  9. District Coaches-in-Chief may check with the local association / affiliate for any additional information regarding the coach requesting the temporary card. The District Coach-in-Chief makes the final decision regarding all Temporary Coaching Card requests.
Although there is a system of checking which coaches have fulfilled their requirements, the responsibility of whether or not a coach has completed the necessary requirements lies with the coaches themselves.
