Contact the registrar of the club you wish to transfer to

Provide the registrar with the required information

Registrar will complete the form and arrange for you to sign or send form to you

Sign the form and submit prior to the deadline. It is your responsibility to make sure the form is submitted prior to the deadline.

Contact Missouri Hockey to schedule your counseling session. Missouri Hockey will not contact you. It is your responsibility to make sure the counseling session is scheduled prior to the deadline.

Attend counseling session. Failure to attend will result in denial of the request.
You must obtain the "One Choice" form from the club you are requesting to transfer too. Please request your "One Choice" form from the Tier II club registrar. You will need to provide the registrar with the following information concerning your child.

Players at the age of 8 and under (Mites and Mini Mites) and Squirt (10 and under) are not eligible to exercise the One Choice transfer.
Once you have received the "One Choice" form and have acknowledged you understand the One Choice rule by providing the appropriate required signatures; you will copy and forward the completed original form to the Missouri Hockey Office (Address on the form). Remember, only a completed "One Choice" form will enable the YD Board to consider your request. An incomplete form will automatically disqualify your transfer request. No Faxes or scanned copies will be accepted. Once the YD Board approves your transfer, you cannot reconsider your request. Forms must be submitted to the Missouri Hockey Office by close of business, 5pm CST, on June 8th. The YD Board will meet to approve requests on or shortly after June 15th.
It is the responsibility of the transferring player to contact the Missouri Hockey Youth Division via email to ask any questions that either the player and/or his parents may have regarding their rights and duties under this rule. Once the player and legal guardian have signed and submitted their "Tier II Player Declaration and Acknowledgment Form" and the Youth Division Board has approved it; they are acknowledging that they have no further rights of choice or transfer except by showing of extreme hardship as defined in the rules.
All "One Choice" players are required to attend a counseling sessions. Counseling sessions will held between June 8th and June 15th at the Missouri Hockey office on Gravois road. For those families that are physically relocating, counseling is available between June 8th and five business days prior to August 31st. It is the responsibility of the of the transferring player to contact the Missouri Hockey Youth Division office by email or phone prior to June 8th to schedule a counseling session. Missouri Hockey Youth Division WILL NOT CONTACT YOU.
What is the "One Choice" rule?
This rule gives the districted player who has played a minimum of one year with an affiliate club, a choice to play for another affiliate member club for the remainder of their youth division career.
There are no restrictions on choosing another affiliate member club to play for, granted that the affiliated club you choose is taking in players of your age division. However, you only get "One Choice" and that choice is your last choice. So make that choice with a lot of inquiry, planning and thought. Not the emotion of the moment.
What is the "Reverse OTT" rule?
This rule gives a player who has previously exercised his or her once choice to another club the oppotunity to return to his or her original districted club.
I am a Mini Mite, Mite or Squirt age player. Can I exercise my One Choice Transfer?
No.
Only Pee Wee, Bantam and Midget age players, ages 12 and above, may exercise their once choice transfer. Mini Mite, Mite and Squirt players, ages 10 and under, are not eligible to exercise the once choice transfer.
Does this choice include Tier I St. Jr. AAA Blues?
No.
Choosing to play for the St. Louis Amateur Blues, a Tier I team does not constitute using your one choice. The "One Choice" rule applies only to Tier II affiliate member clubs. Tier I hockey is highly competitive hockey and therefore trying out for this club remains open to all players without any of the districting restrictions of Section F of the MAIHA - YD Rules and Regulations.
Does this choice include Tier II Central States Development Hockey League (CSDHL)?
No.
Choosing to play for one the CSDHL Tier II teams does not constitute using your one choice. The "One Choice" rule applies only to Missouri Hockey Tier II affiliate member club teams. CSDHL Tier II hockey is competitive hockey and therefore trying out for this club remains open to all players without any of the districting restrictions of Section F of the MAIHA - YD Rules and Regulations.
Do I have the option of going back to my "old" club after exercising my "One Choice" because I did not make the "AA" team of the affiliate member club I transferred to?
Yes.
By exercsing the reverse OTT option. Choosing to transfer to another club for purposes of trying out for another affiliate member club’s "AA" team can be a very risky endeavor. Please be reminded that the basic premise of districted hockey is to provide for balanced teams in order to maintain the recreational value implied at that level. If you are seeking a higher level of competition to further develop your hockey skills then seek out a Tier I organization.
Do I still have the option of using the "Pool Player" process?
The "Pool Player" process remains intact for "AA" players only. The " AA Pool Player" process is described in detail with Section F - 5.of the MAIHA - YD "Rules and Regulations". The "One Choice" rule replaces the old "Pool Player" process for both new and cut players.
If I have exercised my "One Choice" and later move to another district will I get another choice?
Yes and NO.
The "One Choice" rule also replaces the old transfer rule under Section F - 7. (Page 15) of the 1998-99 MAIHA - YD Rules and Regulations. However, if you have not exercised your "One Choice" and have moved into another district you may then do so within the time frame set out under Section F. (This is assuming that the affiliate club is taking players at your age division.) It must be pointed out that if you have not exercised your "One Choice" and you move into another affiliate club’s district, you may hold on to your "One Choice" for future use by continuing to play for the affiliate club that you played for prior to the move. If you previously exercised your once choice option to another club, you may exercise the Reverse OTT option.
Must I scheduling a counseling session prior to completing my OTT request?
Yes.
All players and families wishing to exercise their right of choice under the OTT rule are required to attend a counseling session.
